1)  The ADSA is closed until January 5, 2005

2) The reservation form is only for those who have not paid for the yearbook, and want a copy of their own.  If you paid for it, then you have a copy waiting for you at the ADSA (if you had not already picked it up).

3) If you only paid for the hardcopy, but also want the CD-ROM, then please fill out the reservation form for the CD-ROM.

4) If you only paid for the CD-ROM, but also want the hardcopy, then please fill out the reservation form for the hardcopy.

5) Again, please bring your ID (not necessarily the Miriam ID) and a Letter of Authorization (with an ID if you're not picking up the yearbook yourself--same goes for the CD-ROM).
